We provide integrated primary care and behavioral health treatment programs and assist with providing access to financial and educational resources where applicable.

We’re looking for a dedicated Case Manager to support individuals experiencing homelessness across our residential housing and emergency shelter programs. This role blends direct client support, care coordination, and advocacy to help people achieve stability and long-term success.
Onsite, full-time (40 hr/week), Non-exempt position
Pay Rate: $26/hour based on individual skills and experience
Schedule: M-F, 8am-4:30pm

Key Responsibilities:
Provide Direct Case Management
- Conduct intakes, assessments, and individualized service planning
- Meet regularly with residents to track progress and achieve goals
- Provide counseling, crisis intervention, and advocacy
- Assist with income, benefits, and housing transitions
- Connect clients to community resources and services
- Coordinate care across residential, shelter, and social work programs
- Work closely with internal teams to support client outcomes
- Participate in case reviews and community meetings
- Maintain accurate documentation and HMIS data
- Ensure compliance with agency and funding requirements
- Protect confidentiality and maintain professional boundaries
- Transport clients to appointments as needed using agency vehicles
- Bachelor’s degree in Human Services or related field plus 2–3 years’ experience in homelessness or human services
- Strong communication, organization, and problem-solving skills
- Experience with data systems and reporting (HMIS a plus)
- Valid NYS Driver’s License and reliable transportation
