We provide integrated primary care and behavioral health treatment programs and assist with providing access to financial and educational resources where applicable.

A thorough intake is completed to assess immediate needs, eligibility for programs, and connection to area service providers. Through consistent follow-up, fostering and cultivating trust, and developing relationships, our clients receive the support network they need to better their livelihood.

RSP Case Manager (Rental Supplement Program)
Onsite, Full-time, Non-exempt position
Pay Rate: $24-26/hour 
Schedule: M-F, 8am-4:30pm (40 hrs/week)

The RSP Case Manager provides intake/assessment, crisis intervention, essential/emergency case management, homelessness prevention, rapid re-housing, outreach and referral, and information services. This position will work collaboratively with the Program, Shelter, Residential, and Clinical: Medical Care, Social Work, PNP departments to ensure comprehensive services are provided to each individual in need.  

Key Responsibilities
  • Manage all aspects of the Residential Support Program (RSP), including client eligibility, funding administration, and compliance.
  • Conduct client intakes, assessments, and application reviews to connect individuals and families with housing assistance and eviction prevention resources.
  • Monitor program funding, subsidy utilization, and client outcomes to ensure effective use of resources.
  • Maintain accurate client records and timely data entry in HMIS and other tracking systems.
  • Provide housing navigation, supportive counseling, and referrals to community resources that promote long-term housing stability.
  • Build and maintain partnerships with landlords, property owners, and community agencies to expand affordable housing opportunities.
  • Conduct housing inspections and assist clients in securing safe, stable housing placements.
  • Prepare required reports and documentation, including quarterly program reports and funding updates.
  • Collaborate with internal teams and external partners to coordinate services and support positive client outcomes.
  • Represent the agency professionally in community meetings, case conferences, and stakeholder engagements.
Qualifications
  • Bachelor’s degree (or pursuing a bachelor’s degree) in human services or related field.
  • Experience providing services to the homeless or related human services necessary.
  • Knowledge of agencies, programs, and services in the community.
  • Demonstrated ability to effectively engage and support individuals experiencing homelessness, mental illness, substance use challenges, and those who may be difficult to engage.
  • Valid driver’s license (for one + years), ability to safely transport clients as needed to support access to services and housing stability.  
  • Strong computer proficiency, including experience with data entry, tracking systems, and working with financial or program-related data.
  • Ability to manage a full-time schedule in a fast-paced environment while maintaining accuracy, organization, and attention to detail.
  • Skilled in conflict resolution, with the ability to navigate challenging situations professionally and achieve constructive outcomes.
  • Proven ability to prioritize tasks and meet deadlines, including flexibility to adjust work hours when necessary to meet program goals.
  • Commitment to professionalism, consistently representing the agency with integrity, respect, and cultural sensitivity in all interactions.
  • Knowledge of eligibility criteria and application processes for mainstream/entitlement programs helpful.
