We offer hands-on help to address chronic homelessness, mental health, substance abuse, disability, and systematic poverty by offering innovative solutions and proactive services to the most vulnerable in Schenectady County. We provide integrated primary care and behavioral health treatment programs and assist with providing access to financial and educational resources where applicable.

We are seeking a Case Management Supervisor to lead a dedicated team providing housing and support services to individuals experiencing homelessness or housing instability.

The Case Management Supervisor supervises the Case Management Team along with an active caseload, ensuring high-quality services that help clients access housing, healthcare, and critical community resources. This role collaborates closely with clinical staff, residential teams, and community partners to ensure clients receive comprehensive, coordinated care.

This is an onsite, full-time (40 hr/week), exempt position
Salary: $62,353.20
Schedule: 8:00 am - 4:30 pm

This is a key leadership position that helps shape program outcomes and strengthen our community’s safety net. Key Responsibilities include, but are not limited to:

Team Leadership & Supervision: Supervise and support the Case Management Team, providing guidance, coaching, and oversight.
  • Conduct staff evaluations and manage performance expectations, participate in case conferences, and assist with daily operational workflows.
  • Ensure consistency across the team regarding best practices and agency standards for service delivery.
Client Services & Case Management: Maintain an active caseload and conduct client intakes and assessments for individuals experiencing or at risk of homelessness.
  • Ensure clients are connected to medical, mental health, and supportive services through strong collaboration with clinical and program staff.
  • Facilitate smooth transitions from crisis services to long-term case management and housing stability.
Housing & Community Partnerships: Serve as the agency’s point of contact for housing and case management services.
  • Develop and maintain strong relationships with local landlords and community partners.
  • Coordinate and manage housing and case management referrals, including those received through Unite Us.
Data, Compliance & Reporting: Ensure accurate and timely entry of client information into the HMIS database.
  • Monitor HMIS data quality and run monthly reports to maintain compliance.
  • Prepare monthly, quarterly, and annual program reports.
Additional Responsibilities
  • Participate in the agency’s On-Call Team.
  • Provide crisis intervention and coordinate appropriate referrals.
  • Conduct housing inspections as needed.
  • Accompany clients to provider appointments when appropriate.
  • Represent the agency professionally in community meetings and partnerships.

Qualifications
  • Bachelor’s Degree in Human Services or related field required.
  • 2–5 years experience working with individuals experiencing homelessness or in related human services with a minimum of 5 years supervisory experience.
  • Strong leadership, organizational, and communication skills.
  • Experience working with diverse and high-needs populations.
  • Knowledge of housing resources, entitlement programs, and community services.
  • Computer proficiency and ability to work with data systems (HMIS experience preferred).
Additional Requirements
  • Valid, clean NYS Driver’s License for at least one year, with no DWI/DWAI within the past three years.
  • Must pass the agency road test and meet insurability requirements.
  • Must be able to provide reliable transportation to/from work location.
  • Ability to work full-time during normal business hours, with flexibility to work additional hours as needed.
